FAQ - What You Need to Know to Play High School Sports
Current Physical Examination (AIA Form):
- Required once every school year or after March 1 of the previous school year.
- The form can be obtained online or from the office.
- The form must be signed by a parent/guardian, giving consent for emergency treatment in case of injury and permission to participate in athletics activities.
- The physical examination must be performed by a licensed doctor of medicine (MD), osteopathic physician (DO), certified registered nurse practitioner (NP), or certified physician's assistant (PA-C) registered by the Joint Board of Medical Examiners and the Osteopathic Examiners in Medicine and Surgery.
Current Insurance Coverage:
Students must be covered by accident insurance or have the signed waiver on file.
Registration must be completed online at RegisterMyAthlete.com before a student is allowed to participate in any in-season activity. The registration process will involve filling out necessary forms and providing required information.
AIA Brainbook and Opioid Courses:
Athletes must complete the AIA Brainbook and Opioid courses. These courses can be submitted online or to the Athletics department. The courses need to be completed once in the athlete's MHS career.
Grades and Eligibility:
Athletes must receive passing grades in all classes to meet eligibility requirements. No Pass/No Play policy applies.
Grade checks occur every three weeks.
Transfer Students:
Students transferring into MHS from another high school must complete AIA Form 550 online. For questions regarding eligibility, contact the athletics director.
Participation Fee:
There is a $75 fee assessed for each sport an athlete participates in with a cap of $150 per school year. There is also a cap of $325 per family per school year.

Sports Registration Process
Athletics Clearance Procedure
Step 1: Complete the online registration at RegisterMyAthlete.com.
A parent/guardian must set up an account and complete all of the clearance requirements on Register My Athlete before we can allow their student-athlete(s) to participate in athletics activities.
All student-athletes from the same family should be entered under one parent/guardian account. At no time should there be two accounts for the same student-athlete.
Step 2: Take the Brainbook & Opioid Courses at AZPreps365 Academy. Please note, these courses only need to be completed once during your high school career.
Step 3: Complete and upload the AIA Physical Form. Ensure it's filled out on the most current AIA Physical form for 2023-2024.
Step 4: Complete and upload the Consent to Treat form.
Step 5: Provide proof of insurance or a signed insurance waiver.
Step 6: After completing the above steps, you must obtain a clearance slip form. This form must be given to your coaches prior to any participation.
