Sports Physicals
Sports physicals will take place on Monday, July 25 from 7:00 a.m. until noon in the Mogollon High School office. The cost is $35 and is payable via cash or check. Please make checks payable to Summit. Junior high students must have one physical for the duration of junior high sports. High school students must have a physical every year they participate in high school sports. Please call the office at 535-4622, ext. 2000 with questions, and see our sports physical flyer for more information.
Athletics Clearance Procedures
A parent/guardian must set-up an account and complete all of the following clearance requirements on Register My Athlete before their student-athlete(s) will be allowed to participate in athletics activities. All student-athletes from the same family should be entered under one parent/guardian account. At no time should there be two accounts for the same student-athlete. Please contact your school’s Athletics office should you forget the email or password for the account.
